    Progress in the Research on new Human intercellular information Transmission Mediator--Nitrogen Monoxide transmission Mediator--Nitrogen Monoxide

    Nitrogen monoxide is a very small inorganic compound has recently been discovered to posess very important physiological activities. Multidisciplinary research on endothelium -- derived relaxing factor (EDRF)proved that nitrogen monoxide is wldety syhthesized in the body through L --arginine pathway in the presence of NO synthase. It is now a common understanding that nitrogen monoxide plays the roles as a second messenger as well as a neurotransmltter. It is a very important mediator for intercellular information transmission and widely participates in the regulation of physiological function. Having a very simple structure, nitrogen monoxide plays such an important role In: biological activities. This discovery makes scientists amazed. The research area has been broadened and the knowledge of some diseases has deep ened. As a result,changes in the idea of the pathogenesis and therapeutics of some diseases have been brought about.

    Research on the mechanism Of the action of nitrogen monoxide.will not only help to elucidate the physiological phenomena which are not yet fully understood i but also,=provide new ideas and. approaches to the prevention and if eatment of many diseases.

    The Applfication of Blood Osmotic Pressure Monitoring and m 'is Clinical Significance ' Its Clinical Significance '

    For better understanding of the effect of changes in blood osmotic pressure of burn patients on the course of the disease,we made dynamic observations on 62 cases of burn and found that patients with different seventy and prognosis had different changes in the blood osmotic pressure,including blood colloidal pressUre and crystalloid'pressure,at different stages.

    38 cases in group A had burn area less than 30% BSA, their blood crystalloid pressure did not change significantly in 2 weeks post burn, as compared with that of the healthy people (P>0' 05),while blood colIoidal pressure dropped significantly (P >0.05 ) o 17 cases in group B had burn area larger than 30%BSA, their blood colloidal pressure dropped significantly post burn(p<0. 05),while their blood crystalloid pressure rose significantly (P< 0. 05 ). But compared with that of 7 patients in group O who had burn area larger than so%BSA,the rising amplitude was lower and the rising duration was shorter (P<0. 01). The more serious the case the more significant the drop in blood colloidal pressur and the worse the prognosis. Patients in group C had their blood colloldal pressure dropped continuou8ly,compared with that of group A and B the P valua was respectively<O. 01 and <0.05. The authors analyzed the causes of the change in blood osmotic pressure in order to provide theoretical basis for the application of blood osmotic pressure monitoring in the treatment of burn and complications.

    Clinical Application of MEBO for Treating Thermal injuries

    This paper reports the results of applying MEBO for treating 217 cases of thermal injury. The largest injured area was 60%BSA. Among them, 50 cases had deep second degree wound accounted for 23. 04%. 19 cases had third degree wound accounted for 8. 76%. 95 cases were burned accounted for 43. 77% and 83 cases were scalded accounted for 38. 24%. The patients were treated with MEBO or Jingwanhong ointment. A comparison between the efficacy of the two drugs was made. For superfiCial Second degree burn, MEBO showed slight superiority over Jingwanhong ointment (P >0. 05). For deep second and third degree burns MEBO showed significant superiority over Jingwanhong ointment (P<0. 05). A calculation of the statistically standardized rate of the formation of scar post healing showed that MEBO had a standarized rate of scar formation of 3. 7%, while Jingwanhong ointment had a standardized rate of scar ' formation of 9. 22%. The difference betweens the two groups wss very significant (P<0. 01). Jingwanhong ointment group had a much higher scar formation rate than MEBO group.

    The authprs suggested some points for attention in thecourse of treatment. One patient died from multiple system organ failure and massive hemorrhage of upper digestive tract. 216 patients all cured. After follow6d up for more than 6 months, MEBO proved to leave no scar after healing for deep second degree burns.

    A Survey of Pediatric Septicemia. Post Burn

    This paper reports 214 cases of podiatric septicemia post burn. A survey of the type of bacterial invasion, time for developing septicemia and burn area was done. Ac analysis of the relationship between time for development of septicemia and death rate, complications such ainhalation injury and shock and the age of the patient was made.

    The authors concluded that pwcyanlc and mueti microorganism septicemia is most commonly seen in children. Active management of burn won with timely antiinf6ctive measures is very important especially in 2 weeks post burn and at perioperatlonal period. Effective prevention and treatment of inhalation injury and shock are of great importance in reducing the incidence of septicemia and raising the curative rate.

    Burn Wound PathophySi6logy and ideal Wound Covering Material

    The authors suggested that according to the pathophysiological characteristics of burn wound, an ideals covering material of the wound'should have the following features:

    1. Effectively isolate the wound from external bacterial invasion.

    2. Keep the wound moist.

    3. Reduce body fluid loss.

    4. Maintain a stable internal environment.

    5. Can be used without P6rforming escharotomy.

    6. The drugsapplied on it can penetrate into the wound.

    7. Maintain a constant Supply of the drugs into the wound.

    8. Liquefied materials from the wound can be timely removed.

    9. No adverse effects.

    10. No antigenicity.

    According to their clinical experience of many years, the authors suggested that MEBO is ideal to be ised as burn wound covering material.

    Some Problems in the Prevention and Treatmen of Burns

    This paper presents a discussion on some problems of the prevention and treatment of burna, such hyperplastic scar formation after healing and its harmfulness and introduces recent advahces in the research of this field; The author concluded that the Invention of MEBO provided a remedy for treating burn wound without leaving scar after healing and the development of the new serial tproducts such as MEBO Scar Lotion opened up the way to final solution of this difficult oroblem.

    Clinical Experience with MEBT

    This paper summarizes the experience with MEBT and MEBO in treating 126 cases of different thermal injuries. The total curative rate was 97. 6%. The author concluded that MEBO had analgesic eff6ct. It Improved local blood circulation and prevented fUrther necrosis of the burn tisSue because blood supply was timely resumed. It prevented stasis of blood and eliminated the cause of pain. MEBO also had antiinfective effect because it blocked the supply of nutrient and water to bacteria and created an environment un favorable to bacteria growth. MEBO helped to remove liquefied necrotic tissue and isolated the wound from external infections. MEBO provided a good environment for epithelial regeneration and ther6fore promoted healing of the wound.

    Clinical Report of 386 Cases of Burn Treated with MEBO

    386 cases of burn were treated with MEBO. All the patients healed. The results were very satisfsctory.

    Clinical practice proved that MEBO had good analgesic and antiinfective effects. It improved microclrculation, promoted epithetial proliferation and wound healing. It reduced the formation of scar. For treating residual wounds, MEBO moisturized the wound surface, accelerated the removal of the rotten tissue, promoted granulation and epithellalization. As a result, the wound healed rapidly.

    106 Cased of Burn Treated with Moist Semi--exposed Method.

    106 cases of burn of different depth at different part of the body were treated with MEBO using moist semi --exposed method. The results were satisfactory. Wounds were kept moist with adequate drainage. The isolation of the wound was successful. The frequency of drug change was reduced and pain was lessened. Contamination and infection of the wound were prevented and the amount of the ointment used was reduced.

    Clinical Experience with MEBT in Treating 33 Cases of Burn

    33 cases of burn were treated in the hospital, using MEBT and MEBO. An analysis of 30 cases revealed that MEBT and MEBO had very good analgesic and antiinfective effects. Progressive necrosis of the wound was prevented. No scar formed for deep second degree burn wound after healing. MEBT requires no strict sterilization and is easy and simple to apply. The cost of the medical treatment is much lower than using other therapeutic method.

    MEBT for Treating 19 Cases of Burn

    19 cases of burn were treated with MEBT. The largest burn area was 80%BSA. The depth of burn wound varied from superficial second degree to deep third degree. MEBO was applied in conlunction with a bit of stamp skin grafting for treating deep third degree burn Of large area. Only a little epithelialized scar formed after healing. The scar was soft and the function was normal. Compared with those treated with dry therapy, which always resulted in severe scar formation and even deformity and dysfunction, MEBT had a great superiority over dry therapy.

    Analysis of Treatment Of Burn Septicemia with Large Area Tangential Excision and MEBO

    This paper reports 30 cases of burn septicemia treated with large area escharotomy and MEBO. Operation was performed in a day post septicemia. Necrotic tissue of superficial third degree burn wound was cut away using roller dermatome and covered with MEBO gauze, bandaged with sterile dressing Necrotic tissue of deep third degree burn wound was cut away and covered with artificial skin. 16 cases cured and 14 died. The mean excised area was 27. 6 +12. 48%BSA. For patients with burn wound infected, the operation proved to be very effective. Other necessary treating measures should also be adopted to ensure a good result.

    The Application of Drug and knife Combination Technique of MEBT in Treating Third Degree Burn of the Aged

    19 old patients with third degree burn were treaded from June 1989 to June 1994, with MEBT using drug and knif6 combination technique. The results are Summarized as follows:

    1. The f6ature of third degree burn of the aged is that the liquefication and the reject reaction of wound are weak, and so the healing time of the wound is postponed markedly.

    2. MEBT using drug and knife combination technique can accelerate the liquefication of third dnyee wound, create an adequate drainage and protect the parabiotlc tissue, so that the h68ling time is shortened. It's more suitable for treating third drgree burn of the old patients.

    MEBO Bandaging Method for Treating Pediatric Burn

    50 cases of padiatric small area deep burn were treated with MEBO bandaging method. This method proved to very effective for preventing infections, protecting surviving epithelial tissue, promoting healing and reducing scar formation.

    The authors suggested that:

    1. Some patients had fewer. It was because of inadequate drainage, so frequent and timely removal of liquefied necrotic tissue was necessary.

    2. Antibiotics should be used when wounds were infected by Pseudomonas aeruginosa and Staphylococcus aureus to effectively control the growth of the bacteria.

    All the wounds healed without serious hyperplastic scar formation and deformity.

    MEBT for Treating Pediatric Perineal Burn

    63 cases of podiatric perineal burn were treated with MEBT. The results were very satisfactory. The authors suggested some attention points concerning the difference in the treatment of male and f6male children and for change of posture. The authors concluded that MEBO had good analgesics and antiinfec- bye effects. It lessened the formation of scar. Male children with external genital burn after treated with MEBO,the twounds healed and the physiological function preserved. The children would grow and develop normally. The disablement rate was reduced. This therapy is widely welcomed by the parents.

    The Application of MEBT in the Treatment of Male External Genital Burn External Genital Burn

    130 cases of male external genital burn were treated in the hospital. 85 cases were treated with conventional dry therapy and 45 cases werle treated with MEBT. The results revealed that MEBT was superior to conventional dry therapy. The difference was of statistical significance.

    Clinical Observation on Lip Burn Treated with MEBT

    165 cases of lipbum were treated with MEBT. The wounds were debrieded at the early stage and MEBO was applied. After removal of necrotic tissue, MEBO gauze was used and a semi --e-cpcs6d method was adopted. Application of MEBO gauze continued until the wounds healed. 2 of the patients with superficial third degree lip burn were subjected to autografting. One patient with inhalation injury died from shock, 164 patients were cured. The curative course was shortened. The treatment was nothing painful. The disablement rate was reduced. The results were satisfactory.

    Clinical Analysis of 42 Cases of Ear Burn Treated with MEBO

    42 cases of ear burn were treated with MEBO. The average healing time for different depth of burn was respectively 7.0 + 1. 5, 15. 5 + 2. 25, and 29. 5+ 2. 25 days. In another group,40 cases of ear burn were treated with gentamycin,The average healing time for different depth of burn was respectively 9. 5 + 1. 75, 19.0+2. 50 and 35. 5+3. 75 days. A comparison of the results between the two groups revealed that MEBO was significantly superior to gentamycin (P<O. 01 ).Scar formation forate and wound infection rate were markedly reduced in MEBO group (P < 0. 05). Aural deformity was avoided. MEBO had fast swelling subsiding effect and promoted healing of the wound and had no adverse effect. The application of MEBO was very simple and easy. MEBO was welcomed by the patients.

    Exposure of Bone and Tendon in Various Joints After Large Area Burn Treated with MEBT

    28 cases with exposure of bone and tendon at various joints after large area burn were treated with MEBT. This therapy overcame the difficulty of the survival of skin graft in bone and tendon andlhe shortage of skin source. MEBT was very simple and easy to apply and gave no more wound to the patients.

    A Comparative Study of the Efficacy of Different Drugs for Treating 125 Cases of Craniofacial Second Degree Burn

    125 cases of cranlofacifal second degree burn were divided into four groups and were treated reSpectively by:

    1. washing with chlorhexidine (1: 2, 000), followed by external application of SD--Ag ointment.

    2. washing with chlorhexidine (1: 2, 000f):, fojIowed by SD--Zn.

    3. washing with chlorhexidine, followed by burn wound--pack.

    4. washing with chlorhexidine alone.

    A comperison between the results of the four groups:

    Wound heafing time Chlorhexidine washing alone= 7. 2 days

    Chlorhexidine + SD--Zn= 8.5 days

    Chlorhexidine + wound--pack= 9 days

    Chlorhexldine + SD--Ag= 9. 2 days

    The group treated with chlorhexidine washing alone had the shortest healing time. The difference was of statistical significance(p<0. 05).

    Wound infection rate

    Chlorhexidine washing alone no infection

    Chlorhexidine + SD--Ag 15. 4% infected

    Chlorhexidine + wound--pack

    13. 9% infected

    Ohlorhexidine + SD--Zn 17. 5% infected

    The difference between the group treated with chlorhexidine alone and the group treated with chlorhexidine + SD -- Zn was very significant (P<O. 01 ). The difference between the group treated with chlorhexidine alone and the group treated with chlorhexidine + SD--Ag was significant (P<O. 05).

    17 Cases of Skin Avulsion Treated with MEBO

    17 cases of skin avulsion were treated with MESO. The healing rate was 100%. The results were very satisfactory. MEBO had reliable analgesic and antiinfective effects. The wounds healed quickly. None of the patients needed to perform skin grafting. All the wounds healed in a abort time without apparent scar formation.

    The Efficacy of MEBO in Treating Lichen Planus

    MEBO has the effect of enhancing immunity of local tissue and is capable of creating an environment in which flora imbalance can be controlled. With this idea in mind,the authors used MEBO for treating 45 cases of lichen planus. The curative rate was 12. 5% for treating 15 cases of smooth type lichen planus. For treating 33 cases of eroS.lve type lichen planus,the curative rate was 87. 5%. The average effective rate was 84. 4 %.

    Clinical Observation and Nursingfof Children with Foot Ulcer Treated with MEBO in C6njunction with TDP Radiation

    This paper r6Ports the clinical observation and nursing of children with foot ulcer treated with MEBO in conjunction with' TDP radiation. The course of the disease was shortened for more than 15 days,as compared with those treated with routine dressing change method. The healing rate was 100%. MEBO in conjunction with TDP radiation had good efficacy in promoting metabolism, blood circulation and healing of the ulcer.

    A Comparative Study of the Efficacy of MEBO and Nubao Suppository in Treating Cervical Erosion

    The Application of MEMO in Treating Orthopedic Ulcers

    48 cases of orthopedic ulcer were treated with MEBO. The results were satisfactory. MEBO had good analgesic effect. It quickly subsided local swelling and shortened the healing time. The wounds healed with good elasticity of the skin.

    Repair of Hand and Forearm Skin Defect Using Parumbilical Extra--thin Pedicle Skin Flap Extra--thin Pedicle Skin Flap

    The authors used- extra -- thin pedicle skin flap with skin tube pedicle of inferior epigastric artery parumbilical branch and subdermal vascular net for repair Of hand and forearm skin soft tissue defect. Subcutaneous fat near umbilical side was not excised and the parumbllical skin flap was made into skin tube pedicle of 5. 0 cm long. The distal side of the flap had the cutaneous fat excised while subdermal vascular net preserved. The flap was shaped to the size of the receptor site by trimming. The trimmed area was no more than 70% of the total area of the flap. Cutting bridge of the pedicle was done 10 to 15 days post operation. The pedicle skin tube could be spread and trimmed to repair the remaining area of the wound. The operation should be performed by using microscopic instrument and trimming should be carried out carefully without damaging subdermal vascular net. The flap used for repair should have oozing of bloOd at the distal edge. After operation,the skin flap had good elasticity and good appearance and no encumbrance.
