    The Regulatory Effect Of Cytokine On Pathological Changes Of Burn

    In this paper, the author describes the regulatory effect of cytokine TNF and IL-1 on the pathological changes of burn and particularly, on burn infection, sepsis, multiple organ failure, nutrition and metabolism . The author points out that the use of neutralizing antibody to TNF and administration of IL-1 receptor antagonist would become new treating options because these new treating methods may become an integral part of the care of severely burned patients in near future.

    As an important cellular messenger and active molecule, endogenous nitric oxide has the effect of transmitting and regulating many physiological functions and plays an important role in regulating the functions of cardiovascular and nerve systems and immunological reactions.     54 cases of burn with different area were observed. Their blood osmolality was determined and osmolality discrepancy calculated. Observation was focused on the dynamic changes in blood osmolality of the critical cases . The results indicated that rise in blood osmolality of the morbid cases was accompanied by a marked increase in osmolality discrepancy (OD). The blood osmolality and OD of the morbid cases, while they were living , were higher than that of the surviving cases. Analysis of the data of blood osmolality and the concentration of osmotically dactive substances such as blood sodium , blood urea nitrogen and blood sugar revealed that there are highly positive correlations between these factors. The linear regression equation is :

    Yosm(mOsm/kg.H2O)=49.07+1.656(Na+)+0.878(ureanitrogen )+0.811(blood sugar)or Yosm (mOsm/kg .H2O)=25.74+1.91(Na+)

    This outcome suggested that changes in blood osmolality of large area burn patients can reflect the pathological changes and help to make prognosis.

    As plasm osmotic pressure can be calculated accurately through testing plasma concentration of osmotically active substances , this method is easy and simple and can be popularized to medical units with no osmotic pressure meter.

    The Efficacy Of Anisodamine In The Prevention And Treatment Of Internal Organ Complications Of Sever Burn

    156 cases of burn were treated with anisodamine in combination with routine burn therapy. The results were compared with that of patients treated with routine burn therapy alone . The results proved that anisodamine had good efficacy for preventing and treating internal organ complications was lowered and the curative rate raised.

    Assessment Of Severity Degree And Classification Of Stage Of Eye Burns

    The conventional criteria for assessment of severity degree of burn is not suitable for assessment of eye burns. The authors suggested a set of new criteria for classification of burn position and assessment of the area, depth and degree of eye burn. The authors also suggested methods for recording and classification of clinical stage of eye burn as well as assessment of therapeutic effect of a treating method , in the hope of ophthalmological burn medicine.

    Experience with MEBT in Treating Severely Burned Patients In Groups

    6 groups of 56 cases of severe burn were treated with MEBT in our hospital . The curative rate was 100% and the rate of disablement was 0%. Infection controlling rate was 100%. Successful antishock rate was 100%. In one hospital, within a short period of time, such a large number of severely burned patients treated in groups to give such a satisfactory result is rare in medical history.

    Clinical experience with MEBT in Treating Groups Of Patients With Gunpowder Burn

    16 patients (10 boys and 6 girls, aged from 7 to 12) were burned by gunpowder and were treated with MEBT in the hospital. The burn area was 10 to 30% BSA. 10 patients had deep second degree burn complicated by inhalation injury. All the patients were cured. The results were very satisfactory.

    12 patients were burned at a traffic accident . 5 of them had a total burn area over 50% BSA and third degree burn area over 35% BSA. 5 cases had a total burn area of 20 to 30% BSA and 2 cases below 10% BSA. One of them was complicated by maxilloficial injury. The patients were treated with MEBT/MEBO . Early rescuscitation and antiinfection measures were adopted. All the patients were cured. The results were satisfactory.

    56 cases of moderate and small area burn were treated with MEBO. All the patients healed.Clinical practice proved that MEBO has very little irritation on skin. It has good pain stopping effect and is easy to apply. It has good effect of promoting the healing of electric burn and residual ulceration wounds. For wounds hardly to be exposed, the author used MEBO gauze. The results were very satisfactory.

    This paper summarizes the experience in treating 93 cases of burn using MEBO . MEBO though has on direct bactericidal effect, it does have good infection preventing effect. It forms a thin film after applied on the wound andthus it protects the wounds from infections. MEBO helps to maintain balance of microecosystem of the wounds and promotes the repair of the tissue.

    This paper reports the experience with MEBT in treating 426 cases of burns and wounds. All the patients healed . MEBT is superior to conventional therapy. It has the following advantages.

    1. MEBT can relieve pains and prevent infections.

    2. It makes the wounds easy to be observed.

    3. It prevents progressive necrosis of the stasis zone tissue.

    4. It promotes healing of the wounds.

    5. No scar or very few scars formed after hhealing for deep secong degree wounds. And in particular.

    6. It is very effective ofr treating residual wounds of exceptionally severe burn at the later stage.

    For deep wounds, the authors recommend the use of MEBO in combination with skin grafting in order to achieve early obliteration of the wounds and to reduce the incidence of complicatins.

    36 patients with corneal burn involving 58 eyes were healed by treating with MEBO . The resuls were very satisfactory. MEBO can improve the nutrition supply to the cornea and promote healing of wounds. It can stop pain and reduce irritation and ulceration. The formation of scar can be markedly lessened. The cornea restored with complete physiological function.

    48 Cases Of Electric Burn Treated With MEBO

    48 cases of electric burn were treated with MEBO. The results were very satisfactory . MEBO is suitable for treating electric burn, because the burn wounds often have progressive necrosis and are difficult to debriede when other drugs are applied . MEBO promotes the recovery of parabiotic tissue and protects the surviving tissue to the greatest extent , It has antiinfection effect . Burn wounds treated with MEBO healed with normal physiological function and good appearance. The disablement rate was very low.

    12 Cases Of Pitch Burn Treated With MEBO

    12 cases of pitch burn were treated with MEBO. The smallest burn area was 1.5% and the largest 35% . Upon hospitalization, the patients were treated by spreading MEBO on the wounds after debriedement . The residual pitch was totally peeled off and removed in 2 to 3 days. The treatment was very simple and easy. The treating course was short. MEBO had good analgesic effect. The curative rate was 100%. No complication occurred.

    Patients sustaining pitch burn were treated with either dry or moist therapy . A comparison of the efficacy of the two treating methods was made. The results revealed that MEBO was capable of promoting the removal of residual pitch on the wounds, so that it can prevent pitch poisining. MEBO had antiinfective , pain stopping and healing promoting effects. It is superior to conventional therapy and is easy to apply .

    A Case Of Large Area Burn Treated With MEBO

    A patient with 70% BSA burn was treated with MEBO. Superficial second degree burn accounted for 30%, deep second degree 35% and third degree 5%. The treating course was 50 days . The wounds healed . Clinical practice proved that deep second degree wounds and small third degree wounds can be healed by MEBO with on need of escharotomy and skin grafting . Wounds healed quickly with very few scars. No dysfunction of the joints occurred.

    A Case Of Exceptionally Large Area Pediatric Burn Treated With MEBO

    This paper reports the experience with MEBO in treating a case of exceptionally large area pediatric burn. MEBO had good analgesic effect and promoted drainage of the wounds . Liquefied products were timely removed . MEBO also had antiinfective effect. It protected the surviving tissue to the largest extent and promoted epithelial granulation . No skin grafting was needed and disablement was avoided.

    Application Of MEBT In Home Wards For Burns

    46 cases of burn were treated with MEBT in the home wards. Only 3 cases had bed sore. The results were satisfactory . All the patients were thermally injured but had a relatively small burn area. Most of them were burned in lower limbs and had a relatively small burn area. Most of them were burned in lower llimbs and had difficulty to move about. The establishment of home ward makes it possible for patients to be treated in their homes. The medical expenses can be greatly reduced and man power saved. The author urged to popularize the use of MEBT in home wards.

    Ulcerated Burn Wounds Treated With MEBT

    Residual wounds repeatedly ulcerated at the later stage of severe burn were very difficult to heal, even skin grafting and various antibacterial drugs had been applied .

    This paper reports 22 cases of ulcerated burn wounds refractory to various antibacterials treated with MEBT. The results were very satisfactory . A discussion on the pathogenesis of ulcerated burn wounds, and on the antiinfective , local blood circulation improving and wound healing promoting effects of MEBO was made.

    Diaper Dermatitis Treated With MEBO

    In the period from 1991 to 1994, 80 patients were divided into two groups 40 in each guoup. Patients in one group were treated with MEBO and in the other group with zinc oxide ointment. In the MEBO group, the curative rate was 92.5% , the effective rate are 7.5%, the total effective rate was 100% . In zinc oxide group , the curative rate was 60% , the effective rate was 12.5% , the total effective rate was 72.5%, The difference between the two groups for all the observed items was very significant (p<0.01) . MEBO is non-irritative and has no adverse effect. MEBO is the first drug for treating disease of the kind.

    Application Of MEBO Post-Tonsillectomy

    MEBO was used for treating 120 patients post-tonsillectomy and another 120 cases of post-tonsillectomy were treated with conventional method. A comparison of the results was made . MEBO was superior to conventional drugs for its good analgesic effect , The difference between the two groups was very significant (P<0.01) , The authors made a discussion on the mechanism of the analgesic effect of MEBO.

    58 Cases Of Herpes Zoster Treated With MEBO

    Herpes zoster is commonly seen in the old people . If not treated timely, the vesicles will aggregate to be universal and gangrenous. The case will be more serious. The drugs conventionally used for treating herpes zoster are very irritative and the therapeutic course is very ling. The author recently used MEBO for treating 58 cases of herpes zoster of the aged . The results were very satisfactory.

    Experience With MEBO In Treating Wounds After Excision Of Anal Pointed Condyloma

    10 cases of anal pointed condyloma were subjected to surgical operation. The wounds after operation were treated with MEBO . The results were very satisfactory. MEBO for treating wounds in anus has the following advantages:

    1. MEBO has pain stopping effect. It helps to prevent retention of urine.

    2. MEBO has anti-infective effect and promotes healing of the wounds.

    3. MEBO helps to prevent scar formation and stricture of the anus.

    MEBO for treating large area burn in limbs

    Though efforts have been made to achieve closure of 3rd degree wounds of open fracture , healing by first intention is still difficult to achieve. Secondary infection of the wounds and dryness and necrosis of exposed bone and tendon often occur.

    The authors used MEBO for treating 10 cases of 3rd degree wounds of open fracture . The results were very satisfactory . The wounds healed by first intention.

    MEBO in combination with carbon dioxide laser surgery for treating cervical polypus

    By joint efforts of our two department , 61 cases of cervical polpus were treated with MEBO in combination with laser surgery . MEBO ribbon gauze was applied after excision of polpus with laser . All the patients healded by one treatment only . The curative rate was 100% . This method is very reliable and simple. The patients had no pain and no bleeding. MEBO promoted the physiological repair of the surviving tissue in the wound and prevented bacterial infections. The wounds healed without scar formation . After followed up for half a year, no recurrence was found.

    MEBO In Combination With Carbon Dioxide Laser Surgery For Treating White Lesion Of Vulva

    50 cases of white lesion of vulva were treated with MEBO in combination with carbon dioxide laser surgery. The total curative rate was 10% . At the same time , 50 cases were treated with laser surgery alone and 50 cases with MEBO alone . The total curative rate of the groups were respectively 70% and 60% and recurrence rate 13% and 15% respectively. MEBO in combination with laser surgery was superior. The different between the curative rate was very significant (P<0.01) and the difference between the recurrence rate was also significant (P<0.05). This combined treatment had the advantages of high efficacy , low recurrence rate , no scar formation and no adverse effect.

    Wounds resulted from burns , scalds , surgical operation , trauma and ulceration may form keloid after healing. It is a kind of pathologic hyperplastosis. Patients with keloid have terrible itching and stabbing pain and parching sensation . The patients are suffering from great pains. The authors used MEBO Scar Lotion for treating 28 cases of keloid. The results were very satisfactory .

    This paper summarizes in nursing patients with special and large area burn , focusing on the speciality of electric burn and large area burn. The authors also introduce their experience in antishock fluid infusion , venous puncture and incision , observation on systemic and organ conditions , meatures for prevention of complications , method for application of MEBO , etc. Special nursing attention should be paid to the prevntion of falling from bed and soothing patients who have psychological disturbance.

    Experience In Nursing Burn Children Complicated By Cerebral Edema Due To Excessive Fluid Infusion At The Shock Stage

    Burn children receiving excessive fluid infusion at the shock stage may result in cerebral edema . The onset of the disease is very quick and the prognosis is often not good.

    This paper introduces the experience in nursing 6 cases of pediatric burn complicated by cerebral edema . 4 cases died. The youngest aged 2 and the oldest 8. The average age was 4. The smallest burn area was 19% and the largest 57%. The authors emphasized that the amount of fluid the child received before hospitalizatin should be taken into account and the total amount of infusion should be controlled . It is not permissible to infuse a single fluid in a large amount within a large amount within a short period. Special attention should be paid to the observation of the rescuscitation cindition and etc. At the early stage of cerebral edema, compensatory rise in blood pressure and slow pulse are commonly seen . Points for attention in the treatment and nursing of cerebral edema are also described.
