The Experimental Study on Three--dimensional Architecture of Microcirculatory. Vessels of Human Skeletal Muscle

    (Abstract)

    Bai Shutling Gao Jie

(Dept. of Anatomy ,C.M.U.Shenyang , Liaoning Province )

The morphological characteristics of microcirculatory vascular three-dimensional architecture in biceps muscle of thigh, semimembranous muscle, semitendinaous muscle and gracilis muscle of human body were studied by observation of the vascular casts under scanning electron microscope. The regulation on microcirculatory vascular architecture of human skeletal muscle was also revealed.

We found that the three-dimensional muscular microcirculatory vessel framework was formed by crisscross and overlapping of microcirculatory vessels.

There were not only many parallel and circular microvascular networks in the skeletal muscle but also a lot of vascular loops, loop-like vascular plexus, vascuar networks in concentric circles and fithnet-like networks. They differed from each other in shape and formed a unique intramuscular microcirculatory vascular network.

    Cui Tonglin Wang Baowa

(Dept. of Burns, Affiliated Hospital of Binzhou Medical College and Dept.of Pathologicophysiology, Binzhou Medical College)

13 cases of nail fold burn were treated with treated with MEBT. At the early recovery stage, microangiographic examinations were done using a WX-6 model. multiple position microcirculation video mat. The observation included morphology of the vascular loops. The results proved that MEBT is beneficial to the healing of the wounds and the repair of microvessels.

The instrument can only examine the microcirculation at the nail folds and our clinical experience in this field is limited , we urge that clinicians pay attention to the research on the fundamental field , in order to provide further theoretical evidence of the effect of MEBT on burn wound healing.

(Dept. of Burns, Affiliated Subei Hospital of Yangzhou Medical College)

From January 1981 to October 1993,3,427 burn patients were treated in our department. Among them 136 cases were complicated by invasive systemic infection. Clinical analysis of the complicated cases revealed that the following factors are responsible for the decrease in the incidence of invasive systemic infection:

1.Effective prevention and treatment of severe comlications such as shock, etc.

2.Effective wound management to reduce wound infections and timely excision of eschar and performance of skin grafting for deep burn wounds

3.Improving the systemic condition of the patients and rational use of antibiotics.

(Dept. of Burns, The First Affiliated Hospital , Xinjiang Medical College)

22 burn patients had to perform vein incision because of shock . After failure of percutaneous puncture, the vein was exposed using a trocar and vein puncture was achieved without distal ligation. Permanent blocking of part of the vein was avoided. The higher the ligation position the more the vein blocked.

Application of Moist Therapy in the Treatment of Burns

    (Abstract)

    Kang Bingshan

(First Aid Centre of Beijing Capital International Airport)

Through study and analysis of 9 cases of burn, a conclusion is drawn in this paper that moist exposed burn therapy is the most ideal way in curing burns in hospitals and clinics at grass-roots level.

9 patients, aged from 18 to 61, had 14 burn wounds with the latgest burn area of 42%BSA and second degree burn area of 28%BSA. 8 of them were treated with and healed by moist exposed therapy and 1 of them after 9 days of treatment, transferred to another hospital on his own will.

The author believes that moist therapy is a practical. economical and reliable way in the treatment of burns under various operational conditions. Besides these advantages, the moist therapy is also very effective in relieving pain , avoiding infections. healing up wounds. decreasing the possible scar to minimum, shortening the time required for the treatment and reducing deformity caused by burns.

To the demergency rescue operation(eg. for air crash), which will deal with a large number of the wounded , moist exposed therapy has even greater significance.

Experience with MEBT in Treating 80 Cases of Large Area Burn and Shock

    (Abstract)

    Li Tianyu Zhao Junxiang

    Yang Binhou Jiang Hong

(Nanyang Centre for Burns, Wounds and Ulcers, Henan Province)

80 cases of large area burn complicated by shock were treated with MEBT. The treating course is described as follows:

1.No pain , pain shock was avoided.

2.Wounds were covered with MEBO. Water evaporation was reduced.

3.Wounds were kept in moist environment with no further infury to the wound.

4.Microcirculation was improved and shock was easy to correct. Fluid infusion volume wascalculated according to the formula:

    For adults:Burn area%X body weight(Kg) X 1.6+2500ml

    For children :Burn area% X body weight(Kg) x1.8+ body weight(Kg) X100ml

In the first 24 hours, colloids:crystalloids=1:1. In the second 24 hours , colloids: crystalloids=1:2.1/4 of the total infusion volume was infused in 2 hours post-burn. 1/2 of the total in 4 hours post-burn andother 1/2 was infused uniformly in the later 16 hours.

Patients with slight and moderate shock tided over their shock stage smoothly. Patients with severe shock had to enlarge their blood volume by infusing 20 to 40 ml/kg of normal saline. After correction of shock, fluid infusion was carried out according to the above stated formula in 24 hours post-burn. After 24 hours post-burn, the infusion was carried out according to the formula. For adults, urine outjput was kept above 100 ml /hour and for children 30 ml/hour. Hematocrit was kept below 50%.Timely and rapid infusion is key to the success of shock treatment . Early administration of 654-11, cimetidine and compound red sage root is beneficial to protection of organs from free radical damage and promotion of the efficacy of antishock measures.

The authors also introduced their experience in correcting acidosis and improving microcirculation.

(Dept. of Burns, International Peace Hospital Named After Dr. Norman Bethune)

From 1973 to 1991, 47 cases of severe electric burn were treated in the hospital . Perioperational management was very important which included:

A.Before operation:1.Electrocardiographic examination . Those who had abnormal electrocardiogram had to be treated to avoid accident during operation .2. Resuscitation infusion should be performed early and the infusion volume should be performed early and the infusion volume should be more than that for thermal injury of the same area. 13 patients with shock had fluid infusions. The total amount of fluid infused in the first 24 hours was calculated according to the formula:Burn area(%)Xbody weight(kg)X5 to 7 ml. 3.Decompression fasciotomy should be performed as early as possible. On one hand, pressure could be reduced, and on the other hand, observation of the vitality of deep layer muscular tissue could be made to preserve the length of the extremitles to the graeatest extent.

B.During and after operation:1.Prevention of secondary hemorrhage and suggestion of preventive measures. 2.Before amputation , careful consideration and precaution should be taken. Those who had whole limb necrosis and unrepairable damage of the tissues and severe infections which were life-threatening should be subjected to amputation as early as possible . Those who did not have life-threatening or unrepairable damage should not be subjicted to amputation. Measures should be taken to preserve the limb and to promote recovery of the function . If the stump was short , transplantation of limb could be performed.
