  • A Summary of Clinical Experience with MEBT in Past Three Years

    In past three years, the author treated 1021 cases of burn. Among which 658 cases were treated with MEBT. The results were very satisfactory. The clinical data proved that MEBT was obviously superior to the conventional dry exposed therapy. MEBT had the features of good analgesic effect, good drainage, epithelialization and wound healing promoting effects. For deep second degree and second degree wounds, no scar or very little scar formed. No Skingrafting was needed for superficial third degree wounds. Wound surface evaporation was reduced, so that the volume of'fluid infusion was reduced. MEBO had good antiinfective effect so that systemic dose of antibiotics was reduced.

    MEBT and Burn Wound infection

    Burn wound infection is commonly occurred in burn treatment. The application of MEBT and MEBO markedly reduces the incidence of burn wound infection. This paper reports 61 cases of burn treated with MEBT, the incidence of wound infection was 8.9%. Compared with 28 cases treated with dry exposed therapy and 32 cases treated with bandaging therapy, in the past few years, the wound infection incidence was 32.1% and 37.5% respectively, MEBT gave a much better result. The key to the treatment is to apply MEBO and MEBT in a proper way. This paper also discusses the mechanism of burn wound infection and explains the reasons of the reduction of infection incidence when MEBT is used.

    The Relationship Between the Survival Rate of Skingraf.t and the Time of Operation

    The authors made a survey of the relationship between the survival rate of skingraft and the time of operation postburn and concluded that the optimal time for skin grafting operation is in the period of 10 to 50 days postburn. The results of the operation performed earlier or later than this period are often not very sat isfactory. The reasons of the optimal time for operation were discussed.

    Experience in Treating A Large Number of Patients With Large

    Area Burns Complicated by inhalation injury

    12 patients with a total burn area of 50 to 94% BSA and third degree burn 15 to 62%BSA, were all complicated by inhalation injury. 4 patients with very severe inhalation injury died of respiratory shock and adult respiratory distress syndrome. The mortality was 33.33%. There were many difficulties in the treatment of such a large number of large area burn patients all complicated by inhalation injury. The authors suggested that attention should be paid, first of all, to prevention of shock. Sufficient fluid infusion should be performed to maintain blood pressure at a level above 12/8 KPa and the urine output at a level over 100 ml/ n. Early incision of trachea should be done to improve ventilation and to facilitate resuscitation from shock. Resuscitation fluid infusion should be done at the scene without delay. Second degree burn wounds should be treated with MEBT to ensure spontaneous 1 healing of the wounds. Third degree burn wounds j should be treated with SD--Ag. Escharotomy and autogenous skingrafting should be done properly, group after group, at different times. Early application of antibiotics is recommended to prevent infections and the dose of antibiotics should be large enough, especially during the period of 4 days before and 4 days after operation. Nutrition supporting therapy is also recommended, especially early supply of nutrition through intestinal pathway, in order to maintain the normal functioning of the intestinal tract and to enhance the resistance power of the body, so that infection and complications can be prevented.

    A Case of Exceptionally Large Area Sulfuric Acid Burn Treated with MEBT in Conjunction with Escharotomy and Skingrafting

    A sulfuric acid burned patient had a total burn area of 93%BSA and third degree burn area of 70% BSA, was treated with MEBT in conjunction with escharotomy and skingrafting. The patient healed in 10 days. No occurrence of complications such as septicemia, organ insufficiency and malnutrition had been observed. The patient had one third of his legs amputated. He could not leave the bed, but his mobility was normal while staying on bed.

    256 Cases of Burn Treated with MEBT

    This paper reports the experience with MEBT in treating 256 cases of burn. All, except one, of the patients survived. The healing rate was over 99%. Only 5 cases with third degree wounds had scar formation. The authors suggested that 1. for deep Znd degree and Znd degree wounds, escharotomy and smearing of MEBO should be done repeatedly, to shorten the eschar dissolution period, 2. for deep 2nd degree wounds, MEBO should be smeared continuously for more than 3 months and compression bandage could be applied to prevent scar formation, 3. for shock patients, antishock measures should be taken immediately before debriedement and 4. Iiquefaction product should be removed timely in order to prevent or to lessen general reactions and folliculitis.

    156 Cases of Burn of Late Stage Treated with MEBO

    156 cases of burn of late stage were treated with MEBO. The largest burn area was 25% and the smallest l%. All the patients had second to third degree burns. The application of MEBO started from 3 weeks to 3 months postburn, so the wounds were of late stage. All thevwounds including those with eschar dissolution and those with long unhealed granulation, healed very well. The results were very satisfactory.

    Clinical Observation of 102 Cases of Burn Treated with MEBT

    This paper summarizes the clinical experience in treating 102 cases of burn with MEBO. The author concluded that MEBO had good analgesic, antiinfective and aranulation promotinq effects. The healinq rate was significantly raised by using MEBO as compared with previous experience with other drugs. MEBO is now the best specific remedy for treating burns.

    Clinical Observation of 14 Cases of Decubital Ulcer Treated with MEBO

    14 cases of decubital ulcer were treated with MEBO, in conjunction with infrared therapy. The resuits were very satisfactory. The authors concluded that MEBO had very good and definite clinical efficacy. The wounds healed quickly. MEBO improved microcirculation and promoted spontaneous debriedement. It had analgesic and antiinfective effects. The efficacy of MEBO was enhanced when used in coniunction with infrared therapy.

    The Efficacy of MEBO for Treating Pediatric Diaper, Dermatitis

    152 children with diaper dermatitis were hospitalized. Among which 105 cases were treated with MEBO, while 47 cases were treated with Jingwanhong Ointment. A comparative study of the efficacy of the two drugs was made.

    The efficacy criteria:

    Healed: All the skin lesions disappeared. The color of the skin turned normal.

    Turned better:Obviously lessening of the erosion and erythema, the lesion area reduced and the sign of infections vanished.

    Ineffective: Skin lesion area did not reduce or even enlarged, no improvement of erosion and erythema.

    The results:

    24 hours after application of the drug:

    MEBO group: 10 cases treated with single application of MEBO in the evening, the skin lesion disappeared on the next morning. 34 case6 healed 24 hours after application of MEBO. 58 cases turned better. The total effective rate was 87.6%.

    Jingwanhong group: 1 case healed 24 hours after application of the ointment. 10 cases turned better. The total effective= 23.4%.

    48 hours after application of the drug:

    MEBO group: 69 cases healed. 27 cases turned better. The total effective rate= 91. 4%

    Jingwanhong group: 11 cases healed. 23 cases turned better. The total effective = 72.3%.

    After one course of application of the drug:

    MEBO group: 96 cases healed. 8 cases turned better. The total effective rate= 99%.

    Jingwanhong group: 27 cases healed. 13 cases turned better. The total effective rate= 85. 1 %.

    The difference between the two groups is statistically very significant (P<0.01 ). The efficacy of MEBO was much better than Jingwanhong Ointment. In MEBO group, the children slept very well without crying and the sign of infections disappeared in 24 to 36 hours after application of the drug.

    The authors concluded that MEBO had very good efficacy for treating podiatric diaper dermatitis. It had fast onset of action and good analgesic and antiinfective effects. It had no side effect and was very easy to apply. MEBO is an ideal remedy for this purpose.

    The Nursing and Treatment of A Large Number of Patients with Explosion Injuries

    A large number of explosion victims were treated in our hospital. The treatment had been carried out timely and in an orderly way, even our hospital was not well--equipped with modern medical facilities. MEBO was applied timely. The smearina of the drua the management of the wounds, the nursing and the observation of special parts of wound were done in a proper way. Psychological nursing and sufficient nutrition supply also helped the patient.S.to heal quickly. TI.e results were very satisfactory.

    Nursing Experience in Treating A Child with Exceptionally Severe Burn

    The nursing work for a child with exceptionally severe burn was carried out in accordance with the experience summarized as the followong. The efficacy of MEBO had been made the most. The child healed in 14 days without any complication.

    1. keep the ward clean to prevent cross infection.

    2. At the shock stage, the nursing work should be carried Out with special care. It is very important in successful rescue of the patient.

    3. Good nursing with special attentions to the wounds is very 'important in preventing infections and promoting healing of the wounds.

    4. In the period of infection, good nursing and Sufficient nutrition supply are necessary for prevention of complication.
