    The authors studied the morphological charactertistics of pollical abdominal dermo-reticular cast form and scanning electron microscope. The six forms of glomera are respectively cylindrical , elliptical , globular, cauliflower-like, triangular and irregular.

    The authors abserved the tree-dimensional architecture of the microvessels in nail matrix of adult and found that there are two overlapped layers of vascular net .The detailed architecture of pollical vasculqr tree were studied . The causes of the formation of microvascular architecture and its clinical significance were discussed .

    Treatment of reperfusion Injury of Burn Shock

    70 cases of severe burn shock with reperfusion injury were treated in our hospitals. During the period from 1989 to 1991, 40cases (as a control group )were treated with fluid resuscitation alone and during the period from 1992to 1994, 30 cases were treated with fluid resuscitation in anti-oxyen free radical treatments. The results revealved that in the control group the incidence of reperfusion injury was 57.5% (33/40) and curative rate 77\5%(19/40). While in the group treated with fluid resuscitation in combination with comprehensive measures, the incidence of reperfusion injury was 28.7%(8/30),curative tate 90%(27/30)and incidence of severe complications 23.3%(7/30). The difference between the two groups were very significant (P<0.05).

    The authors stressed the importance of comprehensive measures such as anti-oxygen free radical treatment. These measures helped to protect cells and to support the function of the organs to prevent complications so that the curative rate of burn shock could be increased.

    THE EFFECT OF Anisodamine On The Prevention And Treatment

    Of Burn Shock And Early Injury Of Organ Function

    63 patients with severe burn were treated with prevention of shock and 59 patients with the same disease were treated with fluid resuscitation alone. The efficacy of the two groups was compared. The results revealed that fluid resuscitation in combination with anisodamine had better effect for resuscitation and prevention of early injury of organ function . Anisodamine had good microcirculation promoting effect and it protected the cells and lysosome membrane.

    An Analysis Of The Causes Of Change In Blood Picture

    and Body Temperature At The Early Stage Of Burn

    The authors observed the changes in body temperature and blood picture of 350 cases of burn at the early stage and found that burn patients treated with MEBT had their body temperature rise and return to normal , earlier and quicker than those treated with conventional method. Most of the patients treated with MEBT had their body temperature rise appeared at 4 to 6 hours post burn, the highest could reach 40 .But the patients were in a state of good consciousness and had good appetite . Their body temperature returned to the patients showed that the WBC count amounted to 31.9 to 40×10/L , neutral granulocyte 0.90. Most of the patients upon hospitalization had WBC count of 13 to 18×10/L,neutral granulocyte 0.80 to 0.90. Patients, especially infants , whose WBC count was fewer than 10×10/L at the early stage and had lower body temperature often had unfavorable prognosis. Changes in body temperature and blood picture at the early stage reflect the immunity and stress function of the patient post burn and are closely related to the effect of the drug applied and the treatment of the wound. Proper treatment of the wound helps to control body temperature and blood picture in a favorable range and to be stabilized within a period no longer that a week. Ecological Changes Of Bacteria In Infected Burn Wounds (Abstract) Zhu Xiaoqin (Burn And Plastic Surgery Group, People' Hospital , Maanshan City) Burn wounds provide favorable environment for bacteria growth. Local and systemic infections may occur. During the period from 1987 to 1988 and from 1989 to 1990, the author made investigation two times on the bacteria strains isolated from burn wounds. In 1987 to 1988, among the 74 strains isolated and cultured, 6 strains most commonly seen included Pseudominas aeruginosa, Staphylococcus aureus, Staphylococcus epidermidis, Escherichia coli, Staphylicocus albus and Bacillus subtills . In 1989 to 1990, among the 151 strains isolated and cultured , 6 strains most commonly seen included Staphylicoccus aureus, Pseudomonas aeruginoda , Klebsiella ozaenae, Enterobacter cloacae, Serratia marcesceus and Bacillus subtilis.

    The author suggested that this ecological change of bacteria in burn wound is due to ling term application of sulfadiazine silver and other antibacterial drugs in the hospital and stressed the importance of the prevention and treatment of enterobacterial infections, Enterobacter cloacae and Klebsiella ozaenae in particular . Prevention and treatment of fungal infections are also very important because the detectable rate of fungus in increasing .

    The Influence Of Environment Temperature On The

    Efficacy of MEBT

    MEBT has very good efficacy . But environment temperature has great influence on it , located on cold region , our hospital has very low room temperture in winter , In spring and autumn, room temperature fluctuates greatly. It will give adverse effect on the efficacy of MEBT.

    The author treated 120 cases of burn using MEBT at different room temperature and found that the best room temperature for the drug to display its efficacy is ranging from 26 to 30C , The therapeutic course may be shortened if MEBT is performed at this temperature range.

    Clinical Analysis of Burn Complicated by Hepatitis C

    8 cases of burn were confirmed to be complicated by hepatitis C, by examination of serum anti-HCV. The patients had received blood infusion or blood product. Their blood GPT level was high and fluctuated greatly . The authors found that factors which induced hepatitis included burn shock, infections, operation and hypoproteinemia. Prevention measures should be adopted to protect the liver and attention should be paid to the quality and rational use of blood and blood products.

    64 Cases Of Burn Infected By HCV After Blood Transfusion

    64 out of the 90 burn patients who received blood transfusion , were found to be infected by HCV. The infective rate was 71.1% and the incidence of hepatitis C was 45.6%. The more the blood transfused the higher the infective rate. The author appealed that routine HCV examination should be done to guarantee the quality of blood and blood products for transfusion, in order to lower the rate of HCV infection .

    MEBO gauze was used for treating 84 cases of burns of different kinds. This method cut down the quantity of MEBO used and made the spread of the drug more uniform. The application of MEBO gauze also made it easier to clean the wound. To keep active drainage and to observe the change in the wounds. A detailed technique for the application of MEBO gauze was presented.

    A patient mistakenly swallowed about 50ml of sulfuric acid. His mouth, esphagus and stomach were severely burned . Upon hospitalization , the patient was treated with MEBO at 18 hours post burn. After comprehensive treatment for 14 days , the patient was discharged . MEBO was applied continuously per oral for 2months . The patient was followed up for 10 minths. His mouth and esophagus were nirmal and his stomach proved to be healthy by barium meal examination.

    Digestive tract burn caused by strong acid or alkali is difficult to heal and has no specific remedy and always results in scar formation and narrowing of the tract after healing. The authors used MEBO under the guidance of the principle of MEBT, suggested by Prof. Xu rongxiang, for treating a case of upper digestive tract burn caused by caustic soda. The result was very satisfactory. It solved the problem of narrowing of the digestive tract. MEBT is of first choice for treating digestive tract burns.

    MEBO in Combination With Drug Xoaking Bath For Treating A Case Of Scald of 100% BSA

    A case of 100% BSA scald (superficial second degree 9%, deep second degree 81% and third degree 10%) was healed by treating with MEBO in combination with drug soaking bath. Drug soaking bath together with MEBO played an important role in promoting the healing of large area scald . Large area scald wounds are always infected. Drug soaking bath helped to remove necrotic tissue and to promote adequate drainage in wounds . Therefore, it enhanced the resistance of the local tissue to infections and facilitated healing of the wounds.

    108 Cases Of Specific Ulcer Treated with MEBO

    108 patients suffered from deltoid muscle lateral subcutaneous ulcer in forearm and upper arm after tuberculin test or BCG inoculation were treated with MEBO. Most of the patients were healed in 7 to 15 days with local application of MEBO bandaging. Some of them were healed in 20days . MEBO had good moisturizing effect. It promoted active drainage and facilitated cleaning of the wounds. Removal of necrotic tissue and granulation of the wounds were facilitated. Microcirculation of the local tissue was promoted. Therefore, healing of the wounds was quickened.

    9 Cases Or Refractory Ulcer Of Penis Treated With MEBT

    9 cases of refractory ulcer of penis were treated with MEBT and MEBO . All the wounds healed in 7 to 20days. The method was very simple and easy to perform and had no adverse effect. MEBO promoted adequate drainage and lessened the inflammatory reaction . Microcirculation and local blood circulation were improved and wound healing was facilitated.

    Necrosis and infection of skin flap at the operating site are difficult to handle . Skin transplantation is conventionally considered to be necessary . The author used MEBO for treating such cases , The results were very satisfactory. Infections were quickly controlled and necrosis markedly lessened. The wounds healed quickly . Scar formation was reduced. No skin transplantation was needed. MEBO was strongly effective against pyocyanic infections.

    120 patients with wounds after skin polishing procedures were treated with MEBO to replace the conventionally used vaseline gauze. The results were very satisfactory. The wounds healed quickly . Pigments faded away in a short time . The rate of scarformation was very low. 89 patients were followed up. A discussion on the mechanism of the action of MEBO and indications of the operation was presented.

    Early Microsurgery Treatment Of Sever Thumb Injury

    34 cases of severe thumb injury were repaired and reconstructed with five kinds of microsurgical technique. 16 cases of them were repaired with vascular pedicled island flap. Thumb were reconstructed with antebrachial retrograde island flap in four cases, reconstructed with dorsal antebrachial posterior interosseus artery retrograde island flap in six cases and were reconstructed with transfer of the Second toe in five cases. Only one of 34 cases was failure in transfer of flap, others were successful . Advantages of early repair and reconstruction are shorter hospitalization and earlier functional restoration. As degree of injury and method of therapy are different, each having its advantages and disadvantages. A suitable but simple method is advised to select according to individual conditions.

    Nursing of burn of the aged is a difficult task because old burned persons are week and their conditions are often severe .

    The author summarized the experience in nursing burn of the aged and suggested a careful and considerate nursing programme. Special attention was paid to prevention of complications.

    When MEBT was applied for treating burn of the aged , wounds were directly perceivable and nursing work became easier .

    Nursing experience including antishock fluid infusion , nutrition support , prevention of complications such as pneumonia, decubital ulcer, etc was summarized.
