  • The Effect of Burn Wound Surface Bacteria on Hematogenous Dissemination

    The bacteria species and bacteria density on burn wound surface and in blood culture were detected correlatively in 94 burn patients. 20 species and 984 strains of bacteria were detectable. On wound surface, Gram positive cocci had a higher detectable rate than Gram negative bacilli. In the blood, the detectable rate of the two were at the same level. Staphylococcus aureaus and Pseudomonas auroginosa had the highest detectable rate. The difference between the detectable rates on wound surface and in the blood is probably due to local and systemic application of antibiotics. The bacteria density which may cause hematogenous dissemination differed greatly with different bacteria species. Pseudomonas auroginosa, nitrate--negative bacillus and hemolytic streptococcus had relatively low density. This implied that these bacteria had stronger invasive power and should attract our attention during the course of treatment.

    The Effect of MEBO on Rabbit Skin Vitality

    In this experiment, the activity of succinic acid dehydrogenase (SDH) in rabbit skin was determined to reflect the vitality of the skin.

    Full--thickness of the skin from rabbit back was taken. The skin was cut into cubes, 5 mm3or 10 mm3 in size. In the experiment group, the skin cubes were mixed with different concentrations of MEBO. 8, 15, 30 or 50 skin cubes in one gram of MEBO. In the control group, the skin cubes were stored in normal saline solution. The skin cubes of the two groups Were stored in tightly closed container at a temperature of 20 C. The SDH activity of the freshly prepared skin cubes and the activity on 1, 2, 4 and 7 days after storage were determined respectively in the experiment and the control groups. The results were shown by curves on coordinate paper as the percentage of skin vitality. The results revealed that 5mm3and 10mm3skin cubes of the control and the experiment groups had their vitality percentage curves changed with the same tendency. This implied that MEBO had no significant effect of rabbit skin vitality. Vitality percentage and the mixing concentration were almost positively correlated. The more the MEBO was adhered to the skin cubes, the more serious. the hypoxia of the skin and the more serious the loss of vitality occurred.

    The Three--Dimensional Architecture of Microvessels The Three--Dimensional Architecture of Microvessels of Human Embryonic Organ I. Gall Bladder

    The three--dimensional architecture of microvessels of 24 weeks old human embryonic gall bladder wall was observed using vessel casting mould and scanning electron microscope. In correspondance with the peak--like processes of mucous coat formed by many high and branched rogue walls and gully--like excavations of mucous sinus, peak vascular nets and sinus base vascular nets were also appeared in the microvascular nets of the mucous membrane of the bladder. On the casting mould specimen, vasCular nets were seen around the Archoff holes. Arched bridges were formed in the mucous membrane of the bladder by microvascular anastomosis and also arcuate collecting tubules, microvascular loops and microvascular rings were formed.

    Experience with MEBT in Treating 831 Cases of Burn

    831 cases of burn were treated with MEBT. All the patients healed. The results were very sans-factory. The largest burn area was 39% and the smallest 5%. Among which 498 cases had superficial 2nd degree burns, 209 cases deep 2nd degree and 124 cases third degree. The average healing time was 7. 5 days for superficial 2nd degree, 18. 6 days for deep 2nd degree of the shadow type, 24. 8 days for deep 2nd degree of the deep type and 39. 3 days for third degree. MEBT had very good analgesic and antiinfective effects. No scar or very few scars formed for deep 2nd degree burns. For third degree burns, when the drug was used in conjunction with surgical operation, the healing course could be shortened and scar forms-non reduced. This therapy is very easy to perform and requires no strict sterilization. It is suitable to grass--root medical units in rural areas and in family.

    106 Cases of Pediatric Burn Treated with MEBO

    This paper presents the experience in treating pediatric burn with MEBT, a discussion on the specific characteristics of podiatric burn, an analysis of the problems in conventional treating method and the advantages of MEBT for treating podiatric burns.

    MEBO has good analgesic effect. It promotes liquefication of necrotic tissue and drainage, prevents infection and progressive necrosis of the stasis zone tissue and improves local microcirculation and inhibits excessive regeneration of fibrous tissue.

    MEBO is very easy and convenient to use and is suitable for grassroot medical units in factories and mines, mountain areas, rural areas and construction sites.

    Experience in Treating A Case of Exceptionally Severe Burn involving 95% BSA and Complicated by MSOF

    A patient severely scalded in 120C boiling pool, involving 95% BSA, with 20% superficial Znd degree, 35% deep Znd degree and 40% 3rd degree burns and complicated by severe inhalation injury smoothly passed the shock stage after emergency treatment. 9 days post injury, septicemia with Staphylococcus aureaus and B. colt, respiratory failure and toxic myocarditis happened. After administration of antibiotics in large doses together with expectant treatment, the patient turned better. But on the 12th day post injury, hepatic insufficiency, icteric sclera and electrolyte disturbance occurred. After expectant and liver--protecting treatment, the patient's condition became smooth and the Superficial wounds healed in 19 days post injury. After skin grafting for 3 times, the wounds healed in 85 days post injury. The patient was discgarged from the hospital on the 112th day post injury. Follow--up observations were made for 9 months. The functioning of the four extremities recovered well. There were versicolor scars soft and good in elasticity, in the lower extremities. No joint contracture occurred. The patient was treated with MEBO in the whole treating course.

    Efficacy Analysis of MEBT for Treating 130 Cases of Burn

    An analysis of the efficacy of MEBT for treating 130 cases of burn was carried out. The largest burn area was 80% BSA and the smallest 1. 5%BSA. 24 children and 62 adults had third degree burns. According to the existing classification criteria in China, 5 of them belonged to exceptionally severe burns, 23 severe burns and the rest of the patients moderate or slight burns. All the patients were treated with MEBO and MEBT in the whole course or in part of the treating course. The results were very satisfactory. The therapy had good analgesic and antiinfective effects. It promoted wound healing. For wounds of deep Znd degree or of slighter seventy, no scar or very slight scar formed. 'The author summarized up the experience and made suggestions which may be useful as a reference for clinicians and research workers of the field.

    MEBO for Treating 11 Cases of Bromine Burn

    Bromine burn is a kind of chemical burn. There are some special points for attention in the treatment of bromine burn. Improper treatment of bromine burn may lead to grave consequences.

    The author treated 11 cases of bromine burn using MEBO. All the patients healed. The results were very satisfactory. No scar formed and no bromine intoxication occurred.

    The author recommends that MEBO be the remedy of first choice for treating bromine burn.

    Clinical Experience with MEBT in Healing 13 Cases of Electric Burn

    13 Cases of electric burn were treated with MEBT. The results were very satisfactory. As compared with conventioal therapy, MEBT has the advantages of wide indication, short healing period and low disablement rate. In this paper, two typical - cases were described in detail. The authors concluded that MEBT has the following advantages:

    1. The survival tissue is protected to the largest extent.

    2. Easy inspection of the wounds and promotion of drainage.

    3. Promotion of wound healing.

    4. Lessening scar formation (or even no scar formation) and promoting the recovery of labour ability.

    5. Lessening pains of the patients.

    Experience with MEBT in Conjunction with Dressing Method for Treating Burns

    In addition to moist exposed method, the authors also used MEBO in a moist dressed way, according to the weather, the environment, the position of the burnt area and the age of the patient. 21 cases of burn were treated with MEBO in a moist dressed way. The results were very satisfactory.

    sIn cold winter, it is difficult for many hospitals to carry out exposed therapy. MEBO can be used in a dressed way. The dressing method is very easy to use on extremities and small burn areas, even in family. For children and patients with mental disorder, the dressing method may be more reliable. In grass--root medical units, the dressing method can help to reduce the dose of MEBO and is therefore more economical.

    After debriedement, the wounds including wound edges were smeared with MEBO to a thickness of 3 to smm. A layer of gauze soaked with MEBO was placed on to cover the wounds and 12 to 16 layers of sterile gauze were placed on to a thickness of 5 to 6 mm and then bandaged loosely. The dressing was changed every 24 to 48 hours. The thickness of the ointment apllied and the frequency of dressing change were based on the condition of the wounds and the amount of exudate.

    A Preliminary Survey of the Treatment of Burns Using MEBT

    567 cases of burn were treated with MEBT. The total healing rate was 95. 5%. As compared with the results of dry therapy used in the same period, no significant difference was found between the average healing time and healing rate. But there was statistically very significant difference between the infection rate, complication incidence, frequency of skin grafting for deep 2nd degree wounds and scar formation (P<O. 01). Moist therapy was superior to dry therapy in these aspects.

    The author suggested a policy of the treatment of large area burns namely, ensuring uniform smearing of the ointment, keeping a moist environment, ensuring timely debriedement and thorough exposure of the wounds.

    The author emphasized that scabbed deep wounds should be treated with the ointment in conjunction with surgical operation. Timely removal of the scab would promote early healing of the wound. Active systemic treatment and local application of antibiotics to cover the wound would raise the successful rate of the treatment and reduce the disablement rate.

    A Case of Large Area Burn Healed by MEBT in Conjunction with Escharotomy and Skin Grafting

    A case of burn involving 95%BSA, with 3rd degree burn amounted to 90%, complicated by shock, inhalation injury and stress ulcer bleeding, was treated with MEBT in conjunction with escharotomy and skin grafting. The patient healed. While receiving antishock treatment, the patient was at the same time, treated with MEBO on face, toes and perineal region. After escharotomy and skin grafted for fthe first time, the survival rate of the autoskingraft and alloskingraft was very low, therefore, MEBO was applied onto the operation area. Third degree burn wounds of the other parts of the body were treated group after group with MEBO in conjunction with autoskin grafting. The application of MEBO started on the 6th day post operation.

    The author concluded that while the source of autoskin graft is very limited, this method is recommendable.

    Nursing Care of 200 Cases of Special Position Burns Treated with MEBT

    The author introduced the experience in nursing care of 200 cases of special position burns treated with MEBT. 195 cases had head, face and hand burns and 5 had perineal burn. All the patients healed. General nursing and wound nursing of the special position burns were discussed in detail. A summing--up of the advantages of MEBO was presented.

    Clinical Observation of Chronic Ulcers Treated with MEBO

    Chronic surface ulcers caused by burn, trauma, diabetes, lymphnode tuberculosis, etc. are difficult to heal. A case of ulcer caused by diabetes, had a history of 11 years, did not heal after treated with different methods. The patient was treated with MEBO. The ulcer healed in 37 days.

    This paper also reports 48 cases of ulcers of the kind. The longest history was more than 11 years and the shortest 34 days. The largest area was 12 cm X 10 cm and the smallest 1 cm X 1 cm. The longest healing time was 42 days and the shortest 13 days. After followed--up for half a year, no recurrence happened. The wounds recovered with no apparent scar formation. The results were satisfactory.

    62 Cases of Decubital Ulcer Treated with MEBO

    62 cases of decubital ulcer were treated with MEBO. The results were very satisfactory. Most of the third and fourth degree ulcers could be healed in 60 days. A few serious cases healed in 90 days. After application of MEBO, a series of physical and chemical changes took place. The local immunity and antiinfective power were enhanced. The permeability of the capillaries and the functioning of the microcirculation were improved. A lot of exudate from the wound reacted with MEBO to form something we called"pyric fluid ", created an environment beneficial to the growth and the spreading of granulation tissue and skin island and therefore, promotion of wound healing was achieved.

    The Efficacy of MEBO for Treating 10 Cases of Electric Ophthalmitis

    3 Cases of Acute Stress Ulcer Healed by Oral Administration of MEBO

    Acute stress ulcer of the digestive tract after large area burn is difficult to heal. This paper reports 3 cases of acute stress ulcer post burn.healed by oral administration of MEBO. The efficacy was proved reliable by gastroscopic observations.

    A discussion on the routes of administration and the mechanism of the action of MEBO was made. This result supported the result of a previous paper reporting the efficacy of oral administration of MEBO for treating digestive tract burn caused by caustic soda and created a new route of the administration of MEBO. We hope this will arouse wide attention and further investigations will be carried out.

    98 Cases of Pointed Condyloma Treated with MEBO

    98 cases of pointed condyloma were treated with MEBO. The results were satisfactory. Most of the patients were young people. 44 males and 54 females. Washed the lesion area with aqueous hydrogen peroxide solution and then applied MEBO onto the wart surface and the pedicle. White liquefied substance rapidly formed. After 3 to 4 days, the lesion area was inspected. Application of MEBO continued for 1 to 4 times more, according to how many of the warts fell off. Generally, the disease was cured after 10 to 14 times of application of MEBO. If the size and the number of the warts were very large and a wide area was involved, the warts could be treated group after group.

    89 patients received MEBO 3 to 4 times a day and healed in 4 tb 6 days. 8 patients received MEBO 2 times a day, healed in 5 to 8 days. A case with warts involving a wide area was treated group after group and healed in 15 days.The warts fell off without leaving any scar. No recurrence happened in the followed--up cases.

    2 Cases of Necrotic Erysipelas Treated with MEBO

    The authors used MEBO for treating 2 cases of necrotic erysipelas. The results were satisfactory. MEBO had good analgesic and antiinfective effects. It also had the effects of dilating the capillaries and improving microcirculation. It promoted healing and shortened the therapeutic course. No scar formed. The patients had no pains.

    A Case of Shank Large Ulcer Healed by MEBO

    This paper reports the experience in treating a -- case of shank large ulcer using MEBO. The result proved that MEBO was very efficacious for treating ulcers of the kind. It had very good analgesic and antiinfective effects and promoted drainage and the removal of necrotic tissue, improved local circulation and promoted the regeneration of the skin. The treatment was easy and convenient. The skin after recovery was soft and smooth. No skin grafting was needed.

    Skin Soft Tissue Dilation for Treating Trunk and Four Extremities Burn Scars

    8 cases of burn scars on trunk and four extremities were treated with skin soft tissue dilation. 5 males and 3 females. 3 cases had scars on trunk and 5 on extremities. 7 cases healed. one case had the dilator exposed.

    The authors made a discussion on the attention points during the operation and how to select the dilator.

    The Treatment of Pediatric Hand Scar Contracture Post Burn

    76 children were deformed by hand scar contracture. The case history ranged from 3 months to 6 years. The total number of saer constracture was 122. Among which 71 located on finger, 21 on dorsal part of the hand, 15 on between thumb and index finger and 15 on web--space. 45 scars had slight contracture. 61 moderate contracture and 16 severe contracture. After operation, deformity was corrected on the whole.

    45 slight linear scars were treated with simple "Z " plastic operation and continuous "Z" plastic operation or" Five--valve plastic operation. Moderate scar with tenacity and poor blood supply were excised and operated on using "Z" plastic operation. 30 locations with defect were repaired by transplantation of subdermal vascular net skin flaps. 31 locations by transplantation of moderate thickness skin grafts. 8 severe scars were repaired by skin flaps and & by transplantation of full--thickness skin grafts. Patients were followed--up for 6 months to 2 years. The deformity was corrected on the whole. The recovery of the functioning was satisfactory.

    The author suggested different methods for treating different cases on the basis of his own experience. A discussion was made.

    Experience in Nursing Care of 31 Cases Treated with Axial skin Flap Transplantation

    Skin flap transplantation has recently become an important method in plastic surgery. In Our hospital, 31 patients were treated with axial skin flap transplantation. 30 patients healed. One patient happened to have partly necrosis of the skin flap (amounted to 3. 2 % ).

    The nursing experience was summarized as follows:

    1. Pay attention to psychological nursing. Make the patients free from doubts and misgivings before operation. Straighten the patients' ideas about the operation.

    2. Keep the operation room and the circumstances clean. If infection happens, timely measures should be taken. 3. After operation, the patients should be kept under close inspection to ensure adequate blood supply of the skin flaps, adequate drainage under a negative pressure and strengthen the nursing measures on the skin flap delta region to prevent post operative complications.
